Laminotomy for Removal of Thoracic Ossification of Ligamentum Flavum Presenting Unusual Symptoms

Abstract
Thoracic myelopathy secondary to ossification of ligamentum flavum (OLF) is a rare disease, occasionally presenting unusual and wide ranging symptoms even with no prominent radiological lesions. We report three cases of thoracic OLF which manifested various unusual symptoms, including pain in the lower back, buttocks, legs and neck, sometimes with a tingling sensation or numbness in the fingers or soles. All three patients had single or multi-level thoracic OLF on their diagnostic images. All three were successfully treated with laminotomy and removal of the thoracic OLF. Thoracic OLF should be considered in the differential diagnosis of all patients presenting with unusual symptoms and surgeons should not hesitate to remove the OLF surgically even with no paralytic symptoms.
